Care Navigator – AuthoraCare Collective
AuthoraCare Collective is currently seeking a
Care Navigator
for the
Guide Program
(community‑based care). This full‑time hybrid position supports the Greensboro/Burlington areas with a High‑Point office base. Hours are Monday‑Friday 8:00 AM–5:00 PM, with occasional weekend community events as needed.
Responsibilities
Serve as the main point of contact for beneficiaries and caregivers.
Conduct intake, assessments, and document progress notes in the EMR.
Build strong relationships with beneficiaries and caregivers to promote engagement in medical care and medication adherence.
Identify patient needs in the home or facility and coordinate appropriate resources.
Collaborate closely with the Care Team clinical members for medical, practical, behavioral, or other complex issues.
Maintain a person‑centered care plan and document care outcomes in the EMR system.
Communicate effectively in writing and verbally with AuthoraCare teams, community physicians, and partner organizations.
Qualifications
Graduate of a four‑year social work or human service program (psychology, sociology, human relations).
Ability to establish and maintain effective working relationships with patients, other employees, and the public.
Experience with Microsoft Word, Excel, and HCHB (Homecare Homebase).
Preferred experience working with older adults, caregivers, and seniors with dementia‑related issues.
Current CPR certification.
Valid state‑issued driver’s license, automobile liability insurance, and reliable transportation.
