Brief Description AuthoraCare Collective is currently seeking a
PRN Occupational Therapist
for our
Home Health
department supporting our
High Point, Kernersville and Winston-Salem
service areas. Must be able to travel to, enter, and function in the homes of any patient within the service area.
The Occupational Therapist provides services to restore function, improve mobility, relieve pain, and prevent or limit permanent physical disabilities of patients suffering from injuries and disease. Manage the therapy Plan of Treatment for a caseload of Integrated Health patients in collaboration with other members of the interdisciplinary team (IDT). Assess and identify patient problems, establish therapy goals, and design interventions.
Work with physician to establish a treatment plan using a variety of methods, including exercises, stretching maneuvers, hands‑on therapy, and equipment to ease pain and help increase mobility and endurance. Establish and maintain therapeutic relationships, including community support for resources and education. Evaluate and treat hospice patients as needed. Document goals and outcomes of care.

Requirements
Degree from an accredited Occupational Therapy Program
A minimum of six months of occupational therapy experience preferred
Home Health experience a plus
Current and unrestricted OT licensure
Current CPR certification
Valid state‑issued driver’s license required. Must carry automobile liability insurance at limits required by agency and have own transportation.
