Job Description Summary Position Responsibilities

Identifies barriers to care and refers patient/family to nutrition services, social services, integrative medicine, or psycho‑oncology in a timely manner.

Maintains documentation of all patient encounters and completes reporting requirements in accordance with organization standards.

Adapts flexibly to changing patient and healthcare provider needs.

Participates in professional conferences, in‑service trainings, and meetings as requested or with supervisor approval.

Regularly attends disease‑specific and general tumor boards and interdisciplinary meetings (IPCC).

Supports the patient/family through the cancer journey—from pretreatment education, active treatment, survivorship, to end‑of‑life care.

Collects, analyzes, and reports clinical navigator outcome data, metrics, and productivity benchmarks with administration, IT, and cancer registry staff.

Provides patient education to aid informed decision‑making, improve care timeliness, and support multidisciplinary processes.

Collaborates with physicians/APPS and other healthcare team members regarding pathology and molecular laboratory testing requests and results.

Facilitates timely scheduling of appointments, diagnostic testing, and procedures to promote continuity of care.

Qualifications

License/Registration/Certification

None required.

Education

Bachelor’s degree in a related discipline preferred.

Experience

3‑5 years in patient/healthcare relations or a human services field (e.g., nursing, teaching, counseling), or an equivalent combination of education and experience.
