Home Health Registered Nurse Compensation Pay rate:

$48.69 per hour + mileage

Benefits available to full-time employees after 90 days of employment

401(k) with company match available after 1 year of service

Home Health Registered Nurse Highlights Full-time position:

Monday–Friday, 8:00 AM – 4:30 PM EST

On-call rotation

every 3 weeks ($25/day, $50/weekend/holiday)

Coverage area:

Shelby County, TN

Contract assignment with potential for permanent hire based on performance and attendance

Home Health Registered Nurse Responsibilities Assess patient health status and implement individualized care plans

Provide case management, health maintenance, and disease prevention services

Observe, plan, implement, and evaluate nursing care for patients in community-based or small facility settings

Coordinate care across multidisciplinary healthcare teams

Ensure accurate and timely documentation (daily, monthly, quarterly, and annual records)

Communicate patient condition changes to physicians and ensure follow-up

Oversee medication management policies and administration

Supervise LPNs and unlicensed staff as needed

Schedule and follow up on health-related appointments

Coordinate care during hospital admissions and discharges

Provide new employee training and annual health & safety recertifications

Maintain compliance with infection control, safety standards, and regulatory requirements

Wound care experience preferred (training available if needed)

Familiarity with OASIS documentation and Homecare Homebase (HCHB) preferred

Home Health Registered Nurse Requirements Active

Registered Nurse (RN) license

(compact license accepted)

BLS certification

required

T-Step TB test

required

Home health experience required

Commitment to mandatory attendance during the first 90 days

Ability to participate in on-call rotation
