Responsibilities
Overseeing the admission systems, controls and alliances with healthcare networks
Working with Community Liaison and hospital discharge planners
Working with fellow colleagues as a point of contact to offer support and assistance to ensure the admission process runs smoothly
Preparing paperwork, rooms, and staff for new admissions in both the long‑term care, memory care, and our outstanding rehab unit
Doing the assessments for our Assisted Living side of the facility
Working closely with the Licensed Nursing Home Administrator as well as center staff to ensure a smooth, seamless transition into our center
Consulting and coordinating with healthcare team members to assess, plan, implement, and evaluate resident care plans
Performing RN assessment on all new admissions in a timely manner
Managing the nursing admission process, accurate completion of all required paperwork, complete all physician orders, proper notification, and proper documentation
Assessing, planning, and evaluating care delivery systems, while monitoring resident outcomes
Participating in resolution of resident, family, staff compliments, complaints and concerns
Maintaining an “on call” status for emergencies on a rotating basis
Completing and documenting Weekly Assessment tools
Utilizing established standards/criteria to oversee resident care services
Potentially helping our other campuses with their admissions process and systems
Requirements
WI RN license
Admissions experience preferred but willing to train the right individual
Supervisory experience in a skilled nursing facility
Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures, etc., that are necessary for providing quality care
