What You’ll Do
Complete comprehensive nutrition assessments and develop individualized care plans aligned with patient goals

Collaborate with an interdisciplinary care team to coordinate treatment plans and improve continuity of care
Document interventions, monitor progress, and adjust care plans based on outcomes and evolving patient needs
What You’ll Bring
Bachelor’s degree in nutrition, dietetics, or related field (Master’s preferred)
Registered Dietitian credential (CDR required)
Outpatient or chronic disease experience preferred
