Pay range Base pay: $74,193 – $94,121 per year.
Duties
Providing crisis intervention, service linkages, and discharge planning.
Developing treatment plans with individuals and the treatment team; monitoring and evaluating progress; modifying plans as needed.
Determining discharge needs and appropriate community resources.
Establishing and maintaining relationships with community providers to ensure continuity of care.
Minimum Qualifications Applicants may qualify under the Hiring for Emergency Limited Placement – Statewide (NY HELPS) program if they are licensed and registered in New York as a Licensed Master Social Worker (LMSW).
State Service personnel may be considered for transfer to a competitive class position if they have one year of permanent competitive or 55‑b/55‑c service in a title eligible for transfer under Section 70.1 of the Civil Service Law.
Additional Comments LMSWs are eligible for shift differential pay: 7% for evening and weekend day shifts; 10% for night shifts.
Background checks will be required.
Position is located at Central New York Psychiatric Center's Adirondack Satellite Unit on the grounds of Adirondack Correctional Facility in Essex County.
Compliance: must maintain license and registration; cannot be listed as an excluded individual or entity on any federal or state Medicaid/Medicare exclusion lists. Enrolment as a Medicare provider with a National Provider Identifier (NPI) may be required.
