Responsibilities
Providing crisis intervention, service linkages, and discharge planning.
Developing treatment plans with individuals and the treatment team; monitoring and evaluating progress; modifying plans as needed.
Determining discharge needs and appropriate community resources.
Establishing and maintaining relationships with community providers to ensure continuity of care.
Minimum Qualifications
Licensed as a Master Social Worker in New York State and currently registered.
For candidates outside or within State Service: eligible under the Hiring for Emergency Limited Placement – Statewide (NY HELPS) program.
For candidates within State Service: must have one year of permanent competitive or 55‑b/55‑c service in a title eligible for transfer under Section 70.1 of the Civil Service Law.
