Job Description
General Responsibilities
Conduct thorough medical assessments to inform patient care.
Design and execute tailored care plans for individual patients.
Evaluate and treat acute illnesses in various settings.
Engage actively in discharge planning to ensure continuity of care.
Perform home visits as needed for patient assessment and support.
Facilitate seamless coordination of care across the full spectrum of services provided.
Contribute to the development and review of clinical policies and procedural guidelines.
Take part in the provider on-call rotation as necessary.
Qualifications
Board Certified Nurse Practitioner (ANCC or AANP) – [REQUIRED]
Current, active Nurse Practitioner license in Massachusetts – [REQUIRED]
DEA Registration to prescribe controlled substances – [REQUIRED]
Certification in Controlled Substances (CDS) – [REQUIRED]
BLS Certification – [REQUIRED]
Relevant experience working with frail or elderly populations – [REQUIRED]
Willingness and ability to travel within the designated service area – [REQUIRED]
Certification Requirements
AANP
ANCC
Basic Life Support (BLS)
Controlled Substance Registration (CSR/CDS)
Drug Enforcement Administration (DEA) registration
State License Requirements
Valid Nurse Practitioner license in Massachusetts
Additional Licensure Requirements
Nurse Practitioner certification with prescriptive authority.
