Weekly Hours: Varies

Salary Range: $24 - $36

Premium Pay Rates: $5/hour premium on all hours worked, $1.25/hour weekend shift differential, $1.25/hour evening/night shift differential (6:00 PM - 6:00 AM)

Mileage reimbursement provided for travel within 150 miles of permanent address

Requirements & Responsibilities

Must support multiple locations within 150 miles of permanent address

Long Term Care Experience (6 months preferred, 3 months required)

Minimum of 30 hours per month, one weekend shift per month

Provides professional nursing care for residents of all ages in long-term care under supervision of RN/AP/Physician

Demonstrates nursing principles and competency in resident care per care plan

Performs testing, evaluates data, administers medications or treatments as ordered

Assesses residents, documents interactions and outcomes, specifies interventions

Contributes to interdisciplinary care team, communicates clearly, maintains confidentiality

Counsels residents, family, and advocates under direction of RN/AP/physician

Stays current with required CEU, competency training, and certifications

Qualifications

Graduate from accredited practical nursing program as Licensed Practical Nurse (Associate degree preferred)

IF position requires IV therapy/management skills: graduation from a Board approved AASPN program with IV therapy in curriculum or completion of approved IV Therapy course

Minimum age 18 years

Completed pre-licensure nursing program with supervised clinical experience across life span or have minimum 400 hours LPN experience at another facility

Unencumbered LPN license with State Nursing board and/or multi-state licensure privileges

Maintain required department-specific competencies and certifications

Complete required annual continuing education units (CEU) and competency training
