Clinical Service Representative II – Ophthalmology
Position Summary
Provides clinical office support including greeting and registering patients, scheduling appointments, verifying insurance benefits and referrals, and collecting co-payments.
Scheduled Hours 40 hours per week
Primary Duties & Responsibilities
Greets patients, assists with form completion; verifies accuracy of patient’s insurance and demographic information; provides HIPAA forms, arbitration forms, and consent‑to‑treat forms.
Collects co‑pays for services; verifies and balances receipts and payments.
Reviews schedule for new patients, updating when necessary.
Calls patients with appointment time reminders, obtaining insurance information and referral forms.
Prepares, distributes, and collects fee tickets; reviews fee tickets for completeness; reconciles fee tickets with daily schedule.
Schedules return patient appointments and patient testing; validates patient parking.
May prepare patients’ charts for daily appointments.
Assists patients with insurance questions regarding the billing process.
Assists staff with sending out reminder cards and other related scheduling duties.
Working Conditions
Normal office environment.
Exposure to blood‑borne pathogens.
Requires protective devices.
Patient care setting.
Direct patient care setting.
Physical Effort
Typically sitting at desk or table.
Typically standing or walking.
Typically bending, crouching, stooping.
Occasional lifting (25 lbs or less).

Education High school diploma or equivalent high school certification or combination of education and/or experience.
Certifications / Professional Licenses
Basic Life Support – American Heart Association
Basic Life Support – American Red Cross
Work Experience Medical Office Setting (2 Years)
Skills Communication, Computer Literacy, Insurance, Interpersonal Relationships, Medical Terminology.
Driver's License A driver's license is not required for this position.
Grade C06-H
Salary Range $17.34 – $25.40 per hour.
