The Licensed Clinical Social Worker (LCSW) provides integrated, high-quality mental health services in a community-focused health center.
Responsibilities
Partner with a multidisciplinary team to assess, diagnose, and treat mental health and chronic conditions using evidence-based interventions.
Coordinate care with community resources, facilitate crisis services, and ensure timely access for patients.
Participate in team discussions, contribute to treatment planning, and support clinic quality improvement efforts.
Maintain licensure, credentials, and engage in ongoing professional development.
Provide services in person, including after-hours and at external sites like schools or shelters, with bilingual Spanish skills required.
