Description
Conduct thorough evaluations for patients in inpatient and outpatient settings while crafting personalized treatment strategies
Implement physical therapy interventions concentrating on mobility, strength enhancement, balance recovery, and overall functional improvement
Work collaboratively with nurses, doctors, case managers, and fellow rehabilitation professionals to achieve optimal patient outcomes
Educate patients and their families on therapeutic goals, transitioning plans, and recommended home exercises
Maintain meticulous documentation of evaluations, treatments, and patient progress in line with regulatory and facility standards
Engage in collaborative care team meetings and interdisciplinary conferences to ensure a well‑coordinated approach to patient care
Education
Earned a Doctorate in Physical Therapy (DPT) or an equivalent credential from a recognized academic institution
Certifications
Required state licensure as a Rehabilitation Therapist / Physical Therapist
CPR certification is preferred
Additional specialized certifications (e.g., geriatric, neurological) are valuable but not mandatory
Skills
Experience in the field is highly desired
Able to treat patients dealing with complex medical and mobility challenges
Excellent communication and collaborative skills are essential
Proficient in utilizing digital charting and clinical documentation tools
