Description
Conduct evaluations for patients in both inpatient and outpatient settings, crafting tailored treatment plans for each individual
Deliver physical therapy techniques that prioritize mobility, strength, balance, and overall functionality
Collaborate closely with nurses, physicians, case managers, and fellow rehabilitation professionals to enhance patient outcomes
Guide patients and their families on therapeutic objectives, discharge planning, and home exercise programs
Maintain thorough documentation of assessments, treatments, and patient progress in compliance with facility regulations
Engage actively in interdisciplinary team rounds and conferences to facilitate coordinated care
Education
Doctorate in Physical Therapy (DPT) or equivalent degree from an accredited institution
Certifications
Current state licensing as a
Physical Therapist
CPR certification preferred
Specialty certifications in areas such as neurology or geriatrics are advantageous, but not mandatory
Skills
Prior experience is highly desirable
Capability to assist patients with a range of complex medical and mobility challenges
Exceptional communication and collaboration skills
Familiarity with digital charting and clinical documentation software is essential
