Pay Rate Range: $30‑35 per hour (rates may vary based on case, acuity, schedule, etc.)

Qualifications
Active, unencumbered LPN license
TB skin test – 2 step, current within last 12 months
Six months of prior hands‑on nursing experience
Reliable transportation
Proficiency in clinical assessments, documentation, and compliance with nursing care policies
Continuing education as required by state
Position Overview The Licensed Practical Nurse (LPN/LVN) is responsible for providing and documenting skilled nursing care under the supervision of a Registered Nurse, following the developed care plan and physician orders for each patient while maintaining confidentiality and professional boundaries.
Essential Job Functions
Deliver and coordinate quality patient care in compliance with physician orders.
Continuously observe and assess patient conditions, reporting changes to the supervisor or physician.
Document all activities, assessments, nursing actions, responses, and coordination of care promptly.
Participate, implement, and update the nursing care plan.
Take appropriate nursing action based on assessment to achieve expected outcomes.
Recognize changes in patient needs and responses that require intervention, implementing care to prevent or reduce risk.
Maintain personal and professional accountability by complying with Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act.
Provide care using infection control measures that protect staff and patient per OSHA standards.
Educate patient and family on disease process, self‑care techniques, and prevention strategies.
Maintain knowledge of nursing competencies by participating in educational programs, continuing education units, internal learning management, and skill evaluations.
Requirements
Graduate of an accredited school of nursing.
Current, unrestricted state license as a Licensed Nurse in the state of practice.
Current CPR certification.
Proficiency in clinical assessments, documentation, and compliance with nursing care policies.
Additional State‑Specific Requirements
South Carolina – One (1) year of pediatrics experience.
California – One (1) year of experience under current nursing license.
Louisiana – One (1) year of experience as a licensed nurse.
Continuing education as required by state.
Preferences
Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
Home health experience.
Other Skills/Abilities

Good organization and communication skills.
Physical Requirements
Proficient in speaking, writing, reading, and understanding English.
