Prairieland is seeking a Case Manager/QIDP to serve Moultrie, Douglas, and Coles counties in Illinois. A bachelor’s degree in a human services related field and one-year cumulative experience providing direct service to individuals with developmental disabilities are required. The experience may be gained through employment, volunteer work, or family care. The schedule is 8:00 a.m. to 4:30 p.m., Monday through Friday, with flexibility to work partially from home. Under the direct supervision of the Program Manager of Prairieland Service Coordination, Inc., the Individual Service Coordinator assesses individual needs and ensures the provision of necessary services through communication, service coordination linkage with provider agencies, service monitoring, and individual advocacy. Job Responsibilities

Serve as an Individual Service Coordinator for up to 23 counties, and may be requested to work outside the caseload geographic area. Schedule and conduct intake/PUNS interviews in response to referrals, gathering initial information as required under the PAS Manual and conducting annual reviews as needed. Complete the PAS process following designated procedures, utilizing assistance from a supervisor as appropriate. Provide linkage to other community services for individuals ineligible for PAS/IC services. Advocate on behalf of the individual and/or family to assist the individual in obtaining services to which they are eligible, ensuring direct involvement in the process. Review the individual's medical, residential, vocational, educational, legal, and financial needs and choices through ISC Visiting Notes as required. Schedule and complete Discovery and Person-Centered Plan processes annually or as required. Accurately document services delivered and maintain individual files per program and agency guidelines. Function in a problem-solving capacity with provider agencies regarding service coordination, following the ISC Conflict Resolution guideline. See all waiver individuals as required and complete program requirements set forth in the waiver manual. Coordinate specialized services for eligible Bogard individuals as needed and submit summary reports to external agencies. Monitor services provided to Bogard class members monthly and advocate for any unmet needs. Submit all required documents to the Program Manager for payroll preparation. Participate in scheduled staff meetings and mandated training sessions as recommended. Alert the Program Assistant of PSCI to problematic issues, trends, or service gaps. Perform other related work as required or requested by the Executive Director and/or Program Manager. Supervisory Responsibilities

None Job Qualifications

Bachelor’s degree in human services required and QIDP eligibility as established by the Department of Human Services. Ability to organize, plan, and follow a schedule on a daily, weekly, monthly, and annual basis. Working knowledge of the developmental disabilities service delivery system, including recipient eligibility requirements and procedures. Effective interaction with professionals, paraprofessionals, social service organizations, and client families. Teamwork ability and self-discipline for independent work from a home office site. Flexibility to work outside the normal workweek. Cooperative orientation toward mutual program objectives set by Prairieland Service Coordination, Inc. Willingness to follow directions, accept constructive criticism, and accept responsibility for own actions. Adherence to the Employee Code of Ethics Guideline through professional communication. Reliable transportation with a valid Illinois Driver’s License, personal auto insurance, and a good motor vehicle record. Physical Expectations
