Job Summary:
Unity Health and Rehabilitation Center is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic team. Responsibilities:
* Provide skilled nursing care to residents, including administering medications, wound care, and vital sign monitoring
* Collaborate with the interdisciplinary team to develop and implement individualized care plans
* Perform physical assessments, obtain vital signs, and document resident conditions
* Participate in patient education and discharge planning
* Communicate effectively with residents, families, and healthcare team members
* Maintain accurate and up-to-date records and reports
Requirements:
* Current LPN license in the state of Florida
* Minimum of 1 year of experience in a long-term care or rehabilitation setting
* Ability to work effectively in a fast-paced environment with minimal supervision
* Strong communication and interpersonal skills
* Compassionate and patient-centered approach to care
* Ability to pass a drug test
