Chet Valley Medical Practice is looking to recruit an Advanced NursePractitioner(ANP) to work 27.5hours per week . The successful candidate will be qualified to MSc level and be expected to see patients within thepractice, or their own homes so own transport is essential. Main duties of the job

The ANP will have expert clinicalknowledge and skills. They will have the freedom and authority to act and makeautonomous decisions in the assessment, diagnosis and treatment ofpatients. TheANP will support the team to enable our patients to receive timely care and negate unnecessary delayin receiving treatment. Our ANPs develop close, long-term relationships with our patientsand work in partnership with them to achieve optimum health. They areautonomous in making decisions based on assessment, diagnosis and theinterpretation of test results. They can independently prescribe appropriatemedication, evaluate or refer to other specialists if necessary. Our ANPs are a vital part of our multi-disciplinary team. The following are the core responsibilities of the ANP in deliveringhealth services. a. Develop, implement and embed health promotion and wellbeing programmes b. Managepatients presenting with a range of acute and chronic medicalconditions, providing subject matter expert advice with the practice settingand their own homes c. Implement and evaluate individual specialised treatment plans forchronic disease patients d. Identify, manage and support patients at risk of developing long-termconditions, preventing adverse effects to the patients health e. Provide advanced, specialist nursing care to patients as required inaccordance with clinical based evidence, NICE and the NSF f. Be aware of duties and responsibilities regarding current legislationand adhere to practice policies and procedures on Safeguarding Adults andSafeguarding Children g. Request and collect pathologicalspecimens. Upon receipt, to process and interpret pathology and other testresults as required h. Maintain accurate clinical records in conjunction with extantlegislation ensuring that SNOMED CT codes are used effectively i. Prioritise health issues and intervene appropriately j. Support the team in dealing with clinical emergencies k. Recognise, assess and refer patients presenting with mental healthneeds l. Support in the provision of vaccination programmes for both adults andchildren m. Be an extended and supplementary prescriber, adhering to extantguidance n. Support patients in the use of their prescribed medicines or over thecounter medicines (within own scope of practice), reviewing annually asrequired o. Contribute to practice targets (QOF, etc.), complying with local andregional guidance p. Liaise with external services/agencies to ensure the patient issupported appropriately (vulnerable patients, etc.) q. Delegate clinical responsibilities appropriately (ensuring safepractice and that the task is within the scope of practice of the individual) r. Understand practice and local policies for substance abuse andaddictive behaviour, referring patients appropriately s. Deliver opportunistic health promotion where appropriate There may be on occasion, a requirement to carryout other tasks, this will be dependent upon factors such as workload andstaffing levels Person Specification

Qualifications

Registered Nurse with Nursing and Midwifery Council (NMC) Masters degree required for qualification post December 2020 refer to RCN Credentialing for Advanced Level of Nursing Practice (ALNP) or, a degree for Advanced Practice Qualification up to December 2020 Qualified Independent Nurse Prescriber on the NMC register Has evidence of working at an enhanced level, as described in the Primary Care and General Practice Nursing Career and Core Capabilities Framework Meets NMC revalidation requirements in accordance with the NMC Revalidation booklet Confirmation of registration with the NMC Qualified triage nurse Teaching qualification ALS and PALS Experience

Understanding of general practice and the wider NHS Experience of practice within the four pillars Experience of prescribing and undertaking medication reviews Experience of infection prevention and control measures Experience of quality initiatives, i.e., benchmarking An appreciation of the new NHS landscape including the relationship between individual organisations, PCNs and the commissioners Clinical Knowledge and Skills

Understanding of the importance of evidence-based practice Ability to promote best practice regarding nursing matters Clinical knowledge and skills including ECGs Venepuncture Requesting and processing pathology tests Advising patients accordingly Diabetes Asthma CHD Immunisations Ability to work within own scope of practice and understanding when to refer to GPs Good clinical system IT knowledge and the ability to record accurate clinical notes Broad knowledge of clinical governance Understanding of safeguarding adults and children Knowledge of public health issues in the local area and issues in the wider health arena Understanding of health promotion strategies Excellent interpersonal, influencing, negotiation and organisation skills with the ability to constructively challenge the view and practices of managers and clinicians Understanding and knowledge of healthcare provision in GP surgeries, QOF and enhanced service Detailed knowledge of national standards that inform general practice (NSF NICE guidelines) Disclosure and Barring Service Check
