Responsibilities Home health position serving Dorchester, Jamaica Plain, Mission Hill, and Roxbury areas Float role with patient visits assigned as needed across the South Region Treats patients of all ages from birth to over 100 years Typical shift is 8:00 AM to 4:30 PM Weekend rotation required once every six weeks Uses Epic for electronic medical record documentation Ability to travel between assigned cases is required Provide physical therapy services in patients' homes for a variety of physical impairments and conditions Collaborate with patients, families, physicians, and allied health professionals to coordinate care Maintain timely and thorough documentation according to agency policy and standards Must comply with 60-mile radius rule for local candidates, with local rate adjustment Scrub or professional attire appropriate for home care settings Public transportation and paid parking options available at most locations Position requires use of UKG/Kronos for timekeeping Flu vaccination is mandatory with no exemptions allowed License required if applicable Requirements Active Physical Therapist license in Massachusetts required; no conditional licenses accepted Minimum of 1 year home care experience required AHA certifications required Experience with Epic EMR system is preferred Must be able to travel between patient locations within assigned areas Flu vaccine required; no religious or medical exemptions permitted Benefits High Pay and Bonuses Medical, Dental and Vision Insurance with Day 1 Options 401(k) Plan Weekly Payroll Deposit Free Online CEUs Generous Housing Allowance Travel and Licensure Reimbursements Non-taxed Per Diem and Subsidy Traveler Rewards and Discounts
