$70,000.00/yr - $72,000.00/yr Under the direction of the Program Director, the Re-entry Program Coordinator plays a vital role in ensuring contract compliance and maintaining the quality of services provided by the St. Johns Re-entry Program. This position supports the mission of St. Johns Community Health by overseeing the daily operations of the Re-entry Team and ensuring high-quality service delivery to patients, staff, and funding partners.
The coordinator is responsible for strategic site coordination, deploying teams based on community needs, and facilitating timely, effective communication with the RISE Director. Key duties also include developing and maintaining operational workflows, managing documentation and data reporting, supporting financial tracking and reporting, and serving as the primary point of contact for team members.
The ideal candidate will have experience working with individuals experiencing re-entry from incarceration, possess strong organizational and communication skills, and demonstrate calm, supportive leadership in a fast-paced, care environment.

Qualifications & Licensure
A minimum of a bachelors degree, or at least two years of relevant experience, including but not limited to work with underserved populations, individuals experiencing re-entry after incarceration., and those with associated needs. A comparable combination of education, experience, and/or training will be considered. Ability to work in dynamic, environments. Ability to work independently as well as collaboratively within a team. Strong organizational and time-management skills. Ability to prioritize multiple and competing tasks effectively. Strong, professional verbal and written communication skills. Bilingual (English/Spanish) preferred.
Responsibilities
Team Operations & Site Coordination
Coordinate daily team schedules based on program priorities and community needs. Develop, implement, and maintain staff workflows aligned with program models. Serve as the first point of contact for team questions or concerns; escalate issues to the Director as appropriate. Provide day-to-day leadership to ensure efficient, accountable, and high-quality care delivery. Collaborate with clinic managers, operations, and facilities staff to support program operations and growth.
Data & Documentation
Oversee accurate and timely patient data entry in eClinicalWorks (eCW). Use Power BI to extract and interpret data for internal use and external reporting. Assist with preparation of monthly, quarterly, and annual grant reports.
Administrative Duties
Submit and reconcile monthly expense reports; manage team credit card purchases with Director's approval. Coordinate with HR/payroll to complete and submit team timecards. Maintain program documentation to support audits, compliance, and quality assurance.
Staff Development & Program Support
Support the Director in tracking and coordinating staff trainings, including Evidence-Based Practices (EBPs). Assist in onboarding new staff and promoting team-building activities. Ensure staff stay current on clinical protocols, workflows, and competencies.
Cross-Team Collaboration & Communication
Work closely with internal teams (e.g., Benefits, CHWs, Medical Providers, Data, Finance) to ensure aligned service delivery and reporting. Attend internal and external meetings as assigned to represent the re-entry Program.
