Provide the full scope of services within the provider’s field of training, including diagnosis, treatment, care coordination, preventive care, and electronic health record (EHR) maintenance, following guidelines on quality management, risk management, infection control, customer service, and safety programs for Access Community Health Network. Compensation Package Productivity and Quality Bonuses Relocation Assistance Loan repayment potential through National Health Service Corps (NHSC) NHSC LRP: Up to $75,000 Full Time for 2 years NHSC SUD-W LRP: Up to $75,000 Full Time for 3 years NHSC LRP or SUD-W LRP: $5,000 award enhancement for clinicians proficient in Spanish HPSA Primary Care score 19 for ACCESS Health Centers, with ACCESS Westside, Madison, and Sinai Health Centers scoring 21 232 Hours of Paid Time Off (PTO) per year & Paid Holidays 40 Hours of CME & $2,000 Reimbursement Malpractice liability provided by the Federal Torts Claims Act (FTCA) Flexible schedules and more… Clinical and Operational Duties Obtain health history and perform physical examinations. Diagnose, treat, counsel, and coordinate care for patients. Review and update problem lists and care plans at each visit. Complete all progress notes within 24 hours of patient visits. Participate in on-call responsibilities and respond within 20 minutes. Prescribe medication per Illinois law and practice guidelines. Follow quality standards and clinical guidelines established by ACCESS. Meet Meaningful Use standards by utilizing certified EHR technology to improve care quality, safety, and efficiency while maintaining privacy and security. Manage EHR In-Basket messages and respond within 72 hours. Ensure clinical activities meet reimbursement guidelines. Provide culturally competent care and involve families in care processes. Address patient and family questions and concerns, and promote self-management. Conduct patient education and reinforce follow-up instructions. Utilize appropriate health education materials and tailor communication to patient understanding. Coordinate with care team and case management for comprehensive care delivery. Transfer patients with higher acuity appropriately and meet severity of illness criteria. PCMH Requirements Coordinate with health center staff to meet patient needs. Participate in huddles and meet productivity standards. Ensure timely and appropriate patient care, involving staff as needed. Ensure treatment and education are carried out according to policies. Provider Development Participate in staff orientation and training. Ensure care team meets internal certification standards. Participate in proficiency testing to maintain care standards. May serve as a preceptor for trainees and students. Adhere to HRSA and FTCA regulations. Perform other duties as assigned. Requirements/Preferences Current PA license in Illinois and maintained CME requirements. Board Certification required. Obtain and maintain a collaborative agreement and DEA license upon employment. Two years of outpatient primary care experience preferred. Knowledge of quality assurance, safety, and infection control standards. EPIC EHR competency preferred. Cultural competency and interpersonal skills essential. Strong organizational skills required. Working Conditions/Equipment Ability to concentrate amid interruptions. Ability to stand, sit, and walk for up to eight hours. Ability to perform physical tasks related to patient exams. Computer/laptop use required. ACCESS is a network of Federally Qualified Health Centers serving community-based health care. Depending on the position, vaccination against communicable diseases may be required, or exemptions may be granted for religious or medical reasons. The salary ranges are approximate; actual compensation depends on experience, education, market conditions, and funding.
