COPE Health Solutions Description The Enhanced Care Management (ECM) Program Manager provides leadership and direction for the organizations care management operations and services, aligning with the organization's mission, strategies, and objectives. Position Details FLSA Status: Exempt Salary Range: $68,000 - $75,000 Reports To: Director of Operations Direct Reports: Community Health Workers, Patient Care Navigator Location:

Regular, Full Time Position Responsibilities
Supervise the care management team. Lead implementation of care management programs to address patient needs, improve quality, and ensure appropriate utilization of services. Design, implement, and evaluate care management programs in collaboration with other departments. Lead projects focused on performance improvement and program transformation. Develop and evaluate pilots, programs, and practices based on evidence-based research and outcomes. Standardize and optimize workflows for models of care being scaled. Create playbooks for care management models for dissemination. Perform patient risk stratification and outreach planning with clinical and operational leaders. Organize stakeholder meetings for care coordination programs. Evaluate market research on care management best practices. Design and evaluate IT solutions supporting care management documentation and monitoring. Lead deployment of administrative case management tracking systems. Core Competencies

Valid California Drivers License. Bachelors or Masters degree in a health-related field preferred. 3+ years of leadership experience. Experience in inpatient, rehabilitation, home care, or managed health plans. Experience in multicultural and community-based settings. Basic computer skills; EMR experience preferred. Benefits
