Description Conduct comprehensive patient evaluations in either an inpatient or outpatient setting while crafting tailored treatment plans Implement targeted physical therapy interventions to enhance mobility, strength, balance, and overall function Collaborate with a multidisciplinary team of nurses, physicians, case managers, and rehabilitation specialists to optimize patient outcomes Educate patients and their families about therapeutic objectives, discharge processes, and post-treatment home exercise regimens Maintain meticulous documentation of evaluations, treatments, and patient progress in line with facility and regulatory standards Engage in care team rounds and interdisciplinary meetings to ensure seamless patient care Education
Doctorate in Physical Therapy (DPT) or equivalent degree from a recognized institution Certifications
Active state licensure as a
Physical Therapist CPR certification preferred Specialized certifications (e.g., in neurological care or geriatrics) are highly regarded but not mandatory Skills
Prior experience is strongly recommended Ability to effectively address the needs of patients with complex medical and mobility challenges Excellent communication and collaborative skills Proficient in utilizing digital documentation and clinical charting systems Benefits
