Job Duties Conduct thorough evaluations of patients in inpatient and outpatient settings, crafting tailored treatment plans Deliver physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function Collaborate with nursing staff, medical practitioners, case managers, and fellow rehabilitation experts to achieve optimal patient outcomes Guide patients and their families in understanding therapeutic goals, discharge processes, and effective home exercise regimens Accurately record evaluations, treatments, and patient progress in line with facility and regulatory standards Engage in care team rounds and interdisciplinary meetings to ensure comprehensive care coordination Education Requirements
Doctorate in Physical Therapy (DPT) or equivalent from an accredited program Certifications Required
State licensure as a
Physical Therapist CPR certification is preferable Specialized certifications (such as neurological or geriatric therapy) are advantageous but not mandatory Essential Skills
Strong preference for previous clinical experience Ability to manage complex patient needs encompassing medical and mobility challenges Excellent communication and collaborative skills Proficiency with electronic charting and clinical documentation systems What We Offer
