Description

Coming October 2025 (Pending Funding): Case Manager - Maternal Home Visiting Program

Washington, DC | Hybrid / On-site The Story

Every mother deserves to feel supported, every child deserves a healthy start, and every family deserves someone in their corner. But for many parents in DC, the journey through pregnancy and early childhood comes with challenges-navigating healthcare, securing stable housing, or managing the stress of raising a newborn without enough support.

That's where you come in.

As a

Case Manager

with the Maternal Home Visiting Program, you'll step directly into homes and lives at their most vulnerable and most hopeful moments. You'll provide trusted guidance, coaching, and advocacy-whether it's teaching safe sleep practices, screening for postpartum depression, or helping a family connect to healthcare, WIC, or childcare resources.

Every visit becomes more than a check-in. It's an opportunity to strengthen parent-child bonds, improve health outcomes, and empower families to chart their own futures. You're not just managing cases-you're shaping brighter beginnings. What You'll Do

Case Manager - Maternal Homes 9... Provide regular home visits with education and coaching on prenatal care, infant health, breastfeeding, parenting, and safe sleep. Conduct assessments (health, psychosocial, developmental, safety) and create individualized care plans. Offer counseling and support for perinatal mood disorders and stress management. Connect families with healthcare, housing, education, employment, and social services. Maintain accurate case notes and track outcomes (prenatal care, immunizations, screenings). Advocate for families in schools, clinics, and community agencies. Participate in reflective supervision, peer case review, and professional development. Qualifications

What You Bring

Strong communication, crisis intervention, and organizational skills. Ability to conduct home visits and work flexible hours. (Preferred) DC licensure (LICSW, LPC, LMFT, RN), bilingual skills, and experience with evidence-based models (NFP, PAT, HFA). Why It Matters

This role isn't just about meeting program requirements-it's about creating moments of reassurance, dignity, and strength for families in DC. Each safe sleep practice taught, each successful connection to care, each moment of encouragement adds up to a lifetime of impact for a child and their family.
