Case Manager, Maternal Home Visiting Program — Coming October 2025 (Pending Funding). Washington, DC | Hybrid / On-site. Level: Experienced. Education: Master\u2019s of Social Work Required. Salary: $60,000 - $70,000. Travel: 10 - 20%. Job Shift: Day. Job Location: Home Office (Headquarters) - DC - Washington, DC. Employment Type: Full Time. What You’ll Do

Provide regular home visits with education and coaching on prenatal care, infant health, breastfeeding, parenting, and safe sleep. Conduct assessments (health, psychosocial, developmental, safety) and create individualized care plans. Offer counseling and support for perinatal mood disorders and stress management. Connect families with healthcare, housing, education, employment, and social services. Maintain accurate case notes and track outcomes (prenatal care, immunizations, screenings). Advocate for families in schools, clinics, and community agencies. Participate in reflective supervision, peer case review, and professional development. Qualifications

Master’s degree required in Social Work, Counseling, Psychology, Maternal/Child Health, Nursing, or related field. At least 2 years of case management or counseling with pregnant women, infants, or young children. Knowledge of maternal/child health, trauma-informed care, and infant development. Strong communication, crisis intervention, and organizational skills. Ability to conduct home visits and work flexible hours. (Preferred) DC licensure (LICSW, LPC, LMFT, RN), bilingual skills, and experience with evidence-based models (NFP, PAT, HFA). Why It Matters
