Washington, DC | Hybrid / In-Community | Expected Start: October 2025 (Pending Funding) The Story

Every number in a report hides a real person. A father working two jobs to provide for his child. A young parent balancing school, diapers, and dreams. A teen on the edge of adulthood, searching for guidance in a world that often feels stacked against them.

As a

Case Manager

with the FORGE Fatherhood Initiative, HEART, and Ready4Life programs, you are the steady hand that helps turn struggle into progress. You won't just connect participants to services-you'll walk beside them as they reclaim hope, confidence, and stability.

One week, you may help a father draft his first résumé. The next, you're sitting in a family's living room building a plan for housing, health, and child care. Each small win-an interview scheduled, a counseling appointment kept, a parent reunited with their child-becomes part of a larger story of transformation.

This role isn't about fixing people. It's about reminding them that they already have the strength to move forward-and helping them find the right tools to do it. What You'll Do Listen and Support:

Build trust through compassionate intakes, needs assessments, and action planning. Plan and Guide:

Develop individualized service plans that focus on father engagement, healthy relationships, and economic stability. Connect and Navigate:

Link participants to housing, jobs, education, parenting programs, legal aid, and mental health supports. Advocate and Empower:

Help clients find their voice in systems like child welfare, workforce development, or benefits applications. Document and Learn:

Keep accurate case notes and use outcomes to improve services. Collaborate and Engage:

Work with community partners, attend team meetings, and contribute to outreach events. Qualifications

What You Bring

Bachelor's degree in Social Work, Human Services, Psychology, or related field (Master's preferred). 2+ years of case management or advocacy experience with fathers, young parents, or similar populations. Experience with fatherhood or parenting programs, DV screening, and workforce partnerships. Strong organizational, interpersonal, and communication skills. Familiarity with DC's social service systems. (Preferred) Bilingual ability and/or lived experience as a father or parenting youth. Why This Matters
