Overview

Co-Occurring Disorder Counselor positions (either 1099 Independent Contractor or W-2 Employee) with multi-entity roles. The opportunity includes working with Lighthouse Behavioral Health Center and Community Wellness Outpatient Mental Health Center, across outpatient mental health and substance use disorder treatment services. The contractor/employee arrangement will determine how compensation and engagement are structured, while duties remain focused on delivering evidence-based treatment for individuals with co-occurring mental health and substance use disorders. Division: Operations | Unit: n/a | Accountable to: Operations Manager 2 Important notes: For 1099 option, you will invoice and be paid separately by each entity and must maintain your own taxes, insurance, and licensing. For W-2 option, you will be employed by the organizations with payroll managed by each entity. Roles assume approximately 40 hours per week across programs, with roughly half the time allocated to each organization where applicable.

Responsibilities Clinical Service Delivery: provide substance use disorder focused individual, group, and family counseling for clients with co-occurring disorders; apply trauma-informed, person-centered practices; maintain fidelity to the agency’s clinical treatment models.

Treatment Planning & Documentation: complete initial treatment plans within required timeframes (e.g., first 7 days); conduct monthly plan reviews; perform initial assessments and regular re-assessments; maintain documentation per CARF, Medicaid, and COMAR standards.

Caseload & Case Management: manage assigned clinical caseload; coordinate with internal staff and external providers; address social determinants of health; ensure crisis intervention and safety planning as needed.

Model Fidelity & Quality Monitoring: follow fidelity guidelines for integrated co-occurring treatment; participate in fidelity reviews, audits, and supervision; track outcomes and client satisfaction; complete required trainings and CEUs.

Scope of Focus: The role spans two organizations under shared leadership with coordinated credentialing and billing arrangements:

Focus #1 – Outpatient Substance Use Disorder Treatment: credentialed under Lighthouse Behavioral Health Center; accountable for delivering evidence-based substance use disorder treatment with documented timelines and model fidelity.

Focus #2 – Outpatient Mental Health Treatment: credentialed under Community Wellness Outpatient Mental Health Center; accountable for delivering evidence-based mental health treatment with documented timelines and model fidelity.

Qualifications & Licensing

Required Maryland licensure in one of the following: LCADC, LGADC, CAC-AD, CSCAD, ADT, LCPC, LGPC, LCSW-C, LCSW, LMSW, LBSW, LCMFT, LGMF, Licensed Psychologist, Registered Psychology Associate, CRNP-PMH, APRN-PMH, CNS-PMH, or Student Intern in one of these disciplines. Education: at least a Bachelor’s degree in psychology, social work, counseling, or related field (Master’s preferred).

Required Competencies

• Knowledge of Medicaid documentation, CARF standards, trauma-informed care • Understanding of DSM-5-TR diagnoses; ability to develop recovery-oriented, measurable treatment goals • Timely documentation and clinical billing compliance; strong written and verbal communication • Ability to work in a team and independently; commitment to equity and culturally responsive care

Work Conditions & Schedule

Work is based in community-based offices or hybrid settings; on-site and telehealth delivery as appropriate; anticipated travel up to 10% for interoffice/site-based needs. Schedule typically 4–5 eight-hour days per week, 8:00 AM–5:00 PM, with distribution of hours across the two programs as described in the job posting.

Compensation & Benefits

Contractor compensation is per service (per session/group/client contact) or W-2 salary as described. For 1099, contractor is responsible for taxes, benefits, and insurance; for W-2, benefits are provided per entity. Invoices due on a weekly cadence; specifics vary by engagement type.
