Job Title: Physical Therapist Profession: Therapy/Rehabilitation Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 full year Adult Home Health Experience License: Must have a valid license in hand Certifications: CPR, BLS Must-Have: Recent Home Health experience Description: Home visits are required for this position. Periodic office meetings will be scheduled. The candidate must have reliable transportation and a current driver's license. Agency-required liability insurance is necessary. The average number of patients seen daily is 5-6. Productivity expectations are set at 30 points per week. Each type of visit and activity, including documentation and travel, holds a different point value. The contractor will be assigned between 5-10 patients per day. Types of cases include Start of Care and Follow Ups. Weekend requirements will vary per location and profession. Discuss specific requirements during the interview. Travel up to 75 miles from home is necessary. Mileage will be reimbursed at the IRS standard rate. The commute to and from the first and last patient is not billable. Local candidates are accepted at the same rate. Pending licenses are not accepted; licenses must be in hand unless they are in the process of being obtained. Some positions may have a Baylor Shift schedule of 4x10s, but this position will follow standard 5x8s schedule Monday to Friday. No guarantee of hours is provided. All documentation must be completed within 24 hours of each patient appointment. Delayed and incomplete documentation will impact billing and patient care. The expectation is that all patients on the schedule will be accepted, regardless of location. Declining patients may result in cancellation of the contract. Floating to another location may be required to meet financial needs if there are low patient census levels. Proficiency in OASIS and familiarity with HomeCare HomeBase EMR is expected. All travelers must have used these systems within the last 6 months or complete a retraining course.
