Job Overview The Care Manager (Designated Care Manager) is responsible for providing the highest quality care and services to a consistent group of residents and their families in our assisted living/long-term care and reminiscence neighborhoods. The Care Manager provides hands-on care and supports residents as outlined in each resident’s Individualized Service Plan (ISP) while maintaining a safe, home-like environment. This role supports Sunrise’s mission to champion quality of life for all seniors in compliance with applicable laws and Sunrise standards, policies, and procedures.
Responsibilities
Resident Care: document and review Daily Log information, participate in ISP development and updates, monitor designated residents during shifts, observe and report changes in condition, respond to calls and bells promptly, assist with continence management and sanitation, engage residents in activities per the ISP, and uphold Resident Rights and Sunrise Principles of Service.
Risk Management & Safety: comply with occupational health and safety regulations, use PPE and safety procedures, report accidents/incidents and unsafe conditions, manage hazards and infectious material handling, and ensure safe storage of oxygen and other equipment.
Housekeeping & Laundry: maintain resident rooms and common areas, manage laundry and dining linens, and complete assigned housekeeping tasks.
Dining Service: assist with meals, ensure a pleasant dining experience, follow dietary guidelines per ISP, observe dining changes, and maintain Sunrise hospitality standards.
Reminiscence Neighborhood: support life skills and enrichment activities, encourage resident participation in daily routines and social events, and incorporate “Creating Pleasant Days” into activities.
Training & Team Engagement: collaborate with the team, participate in meetings and training modules, support engagement initiatives, and comply with infection control and regulatory requirements.
Core Competencies: remain calm in difficult situations, manage multiple priorities, communicate effectively, organize and manage time, exercise good judgment and problem solving.
Experience and Qualifications
High School diploma/GED (as required by state regulations).
CPR and First Aid as required by state regulations.
Must be at least 18 years of age.
Previous experience working with seniors is preferred; desire to serve and care for seniors.
Ability to make decisions in the resident’s best interest; driving requirements may apply for Sunrise vehicles where applicable.
