Sunrise Senior Living Care Manager opportunity (Designated Care Manager) at Sunrise of Burlington
Job Overview
The Care Manager/Designated Care Manager is responsible for providing high-quality care and services to a consistent group of residents and their families in our assisted living/long-term care and reminiscence neighborhoods. The role involves hands-on care, physical and emotional support as outlined in each resident’s Individualized Service Plan (ISP) while maintaining a safe, home-like environment. The Care Manager demonstrates the Sunrise Mission to champion quality of life for all seniors in accordance with laws, regulations, and Sunrise standards, policies, and procedures. Responsibilities
Resident Care: document and learn about residents via Daily Log; participate in ISP development and updates; monitor designated residents; observe and report changes in condition; respond to safety systems and calls; greet guests and respond to inquiries; assist with continence management and sanitation; support family communication; promote independence and dignity; lead/adapt activities of daily living (ADLs) as indicated on the ISP; engage residents in reminiscence and life skills activities; maintain a warm, calm environment; ensure safety regulations are followed; practice Standard Care Precautions; host daily resident activities. Risk Management and General Safety: collaborate with the team to comply with health and safety regulations, PPE, SDS, and lockout/tagout procedures; report accidents and hazardous conditions; manage exposure to bloodborne pathogens and hazardous materials; safe storage of oxygen and related safety practices. Housekeeping and Laundry: maintain and clean resident rooms; manage laundry and linens; keep common areas tidy. Dining Service: assist in dining room setup/clean-up; ensure meals meet resident diets; document dining changes in the ISP; provide room service as needed; maintain safe food handling standards. Reminiscence Neighborhood Specific: engage residents in life skills, daily routines, and related programs; support monthly letters home and family engagement. Training and Team Contribution: participate as a team member; adhere to the Team Member Credo and Sunrise Principles of Service; contribute to engagement programs; attend meetings and trainings; comply with infection control and regulatory training; perform other duties as assigned. Core Competencies: remain calm in difficult situations; manage multiple priorities; communicate effectively; strong organizational and time management skills; exercise good judgment and problem-solving. Experience and Qualifications
High School diploma or GED (per state/provincial regulations) CPR and First Aid as required by regulations Must be at least 18 years old Previous experience with seniors preferred Desire to serve and care for seniors Ability to act in residents’ best interests; for drivers, sign the Driver job description if applicable About Sunrise
