Job Overview
The Care Manager/“Designated Care Manager” is responsible for providing the highest degree of quality care and services to a consistent group of residents and their families in our assisted living/long term care and reminiscence neighborhoods. The Care Manager provides hands on care and physical and emotional support as outlined in each resident’s Individualized Service Plan (ISP) while maintaining a safe and comfortable home like environment. The Care Manager demonstrates the Mission for Sunrise Senior Living, “to champion quality of life for all seniors” in accordance with federal, state/provincial, and local laws and regulations and Sunrise Senior Living standards, policies, and procedures to promote the highest degree of quality care and services to residents.
Responsibilities Essential Duties
Resident Care: Review, read, notate, and initial Daily Log to document and learn about residents; participate in ISP development and monthly updates; review designated assignments; be responsible for a designated group of residents during the shift; observe, report, and document changes in residents’ conditions; attend daily Cross Over meetings; notify supervisor of increased care needs; greet guests and respond to calls in a timely, courteous manner; assist with continence management and sanitation; communicate with families; help residents maintain independence and safety; participate in ADLs and life enrichment activities; engage residents in life skills; maintain warmth and calm environment; follow safety regulations; practice Standard Care Precautions; host and engage in daily activities.
Risk Management and General Safety: Ensure compliance with occupational health and safety requirements; practice PPE, fire safety, SDS, and lockout/tagout procedures; report accidents and hazards; keep pathways clear and safe; manage exposure to bloodborne pathogens and hazardous materials; ensure oxygen tanks are stored safely.
Housekeeping and Laundry Services: Maintain resident rooms and common areas; handle resident laundry; manage dining room linens; complete assigned housekeeping tasks.
Dining Service: Serve and assist in dining rooms; promote pleasant dining experiences; assist with setup/cleanup; follow diets as indicated; monitor dining changes; provide room service as needed; practice safe food handling.
Reminiscence Neighborhood Responsibilities: Encourage life skills activities and attendance at social events; incorporate “Creating Pleasant Days” into daily routines; integrate residents’ life skills into daily activities; blend multisensory experiences; participate in monthly letters home with residents and families.
Training and Team Contribution: Work toward team goals; demonstrate Team Member Credo; support residents through Principles of Service; engage in team and engagement programs; attend required meetings and trainings; comply with infection control policies; maintain required training; perform other duties as assigned.
Core Competencies: Remain calm in difficult situations; manage multiple priorities; strong written and verbal communication; good organizational and time management; sound judgment and problem solving.
Experience and Qualifications
High School diploma/GED (as required by regulations).
CPR and First Aid as required by regulations.
Must be at least 18 years of age.
Previous experience with seniors preferred.
Desire to serve and care for seniors; ability to make resident-centric decisions.
If applicable, driver duties require signing the Driver job description for safety and regulatory compliance.
