Position Summary The DDA Case Manager plays a vital role in supporting individuals with developmental disabilities in achieving their personal goals, maximizing their independence, and accessing the services and resources they need. This position ensures person-centered planning, coordination of care, compliance with state regulations, and ongoing advocacy for each individual served.
Key Responsibilities
Serve as the primary point of contact for an assigned caseload of individuals receiving services through the Developmental Disabilities Administration (DDA).
Develop, implement, and monitor Person-Centered Plans (PCPs), ensuring they reflect each individual’s goals, needs, and preferences.
Coordinate and monitor the delivery of services, supports, and resources, maintaining strong relationships with families, guardians, providers, and community partners.
Conduct regular home, community, and program site visits to assess satisfaction, safety, and goal progress.
Maintain thorough, accurate, and timely documentation in compliance with DDA requirements and The League’s standards.
Advocate for individuals’ rights, needs, and inclusion within the community.
Collaborate with interdisciplinary teams to ensure holistic, quality care and service coordination.
Stay current on state and federal regulations, DDA policies, and best practices in disability services.
Qualifications
Bachelor’s degree in Social Work, Human Services, Psychology, Special Education, or related field (required).
At least 2 years of case management experience, preferably within DDA or disability services.
Strong understanding of person-centered planning and community-based supports.
Excellent communication, problem-solving, and organizational skills.
Ability to work independently, manage multiple priorities, and meet deadlines.
Proficiency with case management software and Microsoft Office Suite.
Valid driver’s license and reliable transportation (required).
