Licensed Practical Nurse 4
and provide compassionate, resident-centered care to our residents. In this role, you will deliver direct resident care, use leadership skills to support other LPNs, and step in as relief charge as needed. Updated as of July 1, 2025: This position is eligible for Supplement Basic Shift Premium for 24/7 facilities. Employees assigned to a facility that provides direct care to residents on location will receive an additional one dollar ($1.00) per hour of supplemental basic shift premium for each hour of basic shift premium. The supplemental shift premium does not apply to overtime hours. The premium expires on June 29, 2027. These locations provide Medicare and Medicaid nursing home care to Veterans and, in some cases, their spouses or family members. Responsibilities
Serve as the charge nurse for your assigned area and may act as a backup supervisor in the absence of a registered nurse. Update and maintain MDS documentation. Evaluate, prepare, and revise treatment plans for residents based on observations and recommendations from physicians, psychiatrists, psychologists, social workers, and other nurses. Pass medications, administer and monitor treatments, and assess resident condition. Perform care tasks such as suctioning, oxygen administration, catheterization, irrigation, IV and subcutaneous care, colostomy and tracheostomy care, feeding tubes, pumps, and other ordered treatments. Assist with routine rehabilitative and maintenance care. Assist residents with positioning, transferring, and ambulation. Monitor and maintain a safe environment with preventative infection control. Qualifications
Current Washington state practical nurse license or an unencumbered practical nurse multistate license. Two (2) years of experience as a licensed practical nurse. Proficient in English (read, write, and speak). Preferred/Desired
Long-term care experience. Special Requirements
Must pass a DSHS BCCU Background Check prior to beginning services and every two years per WDVA Policy 615.000. Must comply with annual Tuberculin Screening per DVA policy 670.000. Must wear appropriate personal protective equipment and comply with safety standards, including respirator use when required. Must be able to lift up to 60 lbs and meet mobility requirements as needed for stocking and accessing supplies. Must maintain a valid LPN license and regular attendance; must be willing to work as needed. Must comply with all WDVA policies and applicable collective bargaining agreements; sign confidentiality agreements. Positions requiring travel must have a valid unrestricted driver\'s license and be insured for state business use of a private vehicle. Candidates must have work authorization that does not require visa sponsorship now or in the future. Video
