Responsibilities

Administrative Functions

– Assist the Director of Nursing Services in directing the day-to-day nursing activities in accordance with current rules, regulations, and guidelines governing the long term care facility. Develop, maintain, and update written policies and procedures for the nursing service department. Ensure reference materials (PDRs, regulations, professional standards) at nurses\' stations are current. Recommend material to assist the nursing service department in meeting resident needs. Maintain the Nursing Service Procedures Manual and ensure compliance with its procedures. Prepare written and oral reports to the Director concerning the nursing service operation. Periodically review and revise department policies, procedures, and job descriptions. Coordinate nursing services with other resident services to ensure continuity of care. Ensure compliance with job descriptions, participate in quality assurance and facility surveys, and assist in discharge planning as required. Admit, transfer, and discharge residents; complete accident/incident reports; interpret policies to personnel, residents, visitors, and government agencies; perform administrative duties and maintain security and confidentiality of information.

Committee Functions

– Attend facility committee meetings (e.g., Infection Control, Policy Advisory, Pharmaceutical, Quality Assessment) and prepare reports of nursing service programs for those committees. Provide information to the Quality Assurance and Assessment Committee and participate in care plan committee agenda development and discharge plan reviews. Implement committee recommendations as directed by the Director.

Personnel Functions

– Ensure adequate staffing by licensed nurses and certified nursing assistants for each shift. Develop work assignments and delegate authority to Charge Nurses. Ensure all nursing assistants are enrolled in or have graduated from approved training programs and supervise nurse aide trainees. Conduct daily rounds and report findings; monitor absenteeism; enforce policies and dress codes; review grievances; maintain interdepartmental relationships; foster a warm, professional environment; conduct periodic staff evaluations and staffing discussions; participate in disciplinary actions as needed. Address safety, privacy, and security concerns, and report incidents of fraud or unauthorized access to information systems.

Nursing Care Functions

– Inform staff of admissions, room assignments, and ensure rooms are ready. Greet new admissions, accompany them to rooms, and coordinate rounds with physicians. Plan care with residents, families, and physicians; notify changes in condition and obtain timely physician notes and orders. Observe residents daily, ensure direct nursing care by licensed personnel or qualified aides, review nurses\' notes for accuracy and alignment with care plans, perform direct nursing tasks as needed, and manage IVs, specimens, vital signs, and admissions/discharges. Use corrective actions as needed and keep the Director informed through reports. Authorize restraints per policy and supervise medication administration and charting. Inspect treatment areas and ensure safety and cleanliness. Monitor for call-bell responsiveness and communicate with families as needed. Arrange discharge transportation and notify families of resident death.

Staff Development

– Develop and deliver in-service training; support facility in-service programs (OSHA, TB, HIPAA, Abuse Prevention, Safety, Infection Control, etc.); plan clinical supervision for nurse aide trainees; participate in continuing education to maintain licensure.

Safety and Sanitation

– Assist in safety standards development; ensure training for exposure risks; maintain PPE stocks; enforce PPE usage; monitor compliance with safety regulations; maintain clean and sanitary work areas; enforce hand hygiene; conduct fire safety and disaster drills; participate in infection control programs; implement reporting procedures for hazardous conditions; use safety equipment when lifting or moving residents; ensure MSDS availability and labeling; report safety issues.

Equipment and Supply Functions

– Recommend equipment and supply needs; maintain adequate stock levels; assist in preventive maintenance; ensure safe operation of equipment; train personnel; monitor supply usage to avoid waste; maintain MSDS for hazardous chemicals.

Care Plan and Assessment Functions

– Participate in developing resident assessments and plans of care; involve residents in care plan development; review plans for goals and revisions; ensure staff awareness of care plans; reference care plans before care delivery; assist in planning and revising MDS, RAPs, and Triggers; ensure care aligns with planned treatments and resident wishes.

Budget and Planning Functions

– Stay informed of economic conditions affecting nursing services; make recommendations on equipment needs; report suspected fraud related to false billings or cost reports.

Supervisory Requirements

– Assist in supervising and managing nursing staff; RN is delegated administrative authority and accountability for assigned duties.

Qualifications

Education and Experience

– Nursing degree from an accredited college or university; training in rehabilitative and restorative nursing practices; 2+ years of experience, preferably in a long-term care facility.

Certificates, Licenses, Registrations

– Current, active RN license in the state; current CPR certification; knowledge/experience with PCC preferred; continuing education/licensing maintained; good standing with State Board of Nursing.

Language Skills

– Ability to read, write, speak, and understand English; ability to read technical procedures and policy manuals; effective communication with managers and staff.

Mathematical Skills

– Ability to apply fractions, percentages, ratios, and proportions to practical situations.

Reasoning Ability

– Ability to make independent decisions when warranted; solve practical problems; relate information about residents; work with diverse individuals; demonstrate leadership and supervisory ability; plan and implement programs, goals, and procedures; provide age-appropriate care and be a team player.

Physical Demands

– Move intermittently; speak/write English clearly; handle mental and emotional stress; vision/hearing or use of prosthetics; lift a minimum of 25 pounds up to 5 feet and carry for 50 feet; ability to assist in emergencies.

Work Environment

– Office and nursing areas; frequent interruptions; potential exposure to hostile or emotional situations; occasional long hours, weekends, holidays; possible call-backs; exposure to infectious waste, TB, AIDS, Hepatitis B; handling hazardous chemicals; risk of injury from falls and other hazards; ability to plan for residents\' needs and coordinate with others.
