About the Role The Primary Care Physician (PCP) demonstrates accountability for patient outcomes, acts as a health coach, and simplifies care actions to promote behavioral change. The PCP works independently and collaboratively, ensuring compliance with guidelines, participating in risk and quality programs, and contributing to a positive culture of love, accountability, and passion.
Essential Job Duties / Responsibilities
Provide comprehensive care for acute and chronic illnesses in older adult patients, managing assessment, diagnosis, treatment, and documentation.
Exercise full accountability for patient care and outcomes, seeking specialist consultation when necessary while remaining involved in detailed care.
Engage hospitalists for patients admitted to the hospital and collaborate with the transitional care team for homebound or facility-based patients.
Lead a care team consisting of medical assistants, facilitators, and coordinators, ensuring effective care coordination.
Participate actively in center management, cover for colleagues, recruit patients, and aid in recruiting additional providers.
Supervise or collaborate with Advanced Practice Practitioners, maintaining accountability for their actions.
Perform additional duties as assigned by management.
Knowledge, Skills & Abilities
Availability and accessibility to build patient trust, including walk‑in visits and timely communication.
Service orientation, delivering care that reflects what you would want for a family member.
Evidence‑based medicine, with emphasis on lifestyle, nutrition, and holistic interventions.
Leadership development for patients, teams, the center, and the company.
Commitment to quality, continuous improvement, and practice efficiency.
Influence patients and teams to achieve optimal outcomes compassionately.
Self‑care to sustain personal well‑being and deliver high‑quality care.
Proficiency in Microsoft Office Suite and related software.
Willingness to travel locally, and occasionally regionally or nationally for training or recruiting.
Fluency in English (and other languages if required).
Independent judgment and flexibility to work evenings, weekends, or holidays as needed.
Education And Experience Criteria
MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty.
Current or obtainable State Medical License in the relevant state(s).
Board certification or eligibility preferred; ongoing maintenance of certification required.
DEA number for Schedule II–V controlled substances.
Basic Life Support (BLS) certification from the American Heart Association or American Red Cross within the first 90 days.
Pay Range $214,700 – $306,714 salary
