Complex Discharge Co‑ordinator Band 6 £41,608 - £50,702 per annum
Plus Distant Islands Allowance of £1,406 per annum
37 hours per week
Fixed Term Contract/Secondment Opportunity for 12 months
A new and exciting opportunity has arisen for an individual to play a key role in ensuring our patients enjoy as smooth and efficient a journey as possible following admission to the Western Isles Hospital. The successful applicant will work collaboratively and closely with colleagues across health and social care partnerships to both anticipate and plan for the patient journey from admission to discharge. They will act as the main point of contact for patients, their families, and colleagues in tertiary centres seeking to repatriate patients back to the Western Isles hospital for ongoing care.
Responsibilities
Coordinate patient discharge and repatriation to the Western Isles hospital.
Communicate effectively with patients, families, and clinical teams.
Plan and anticipate patient needs considering the acute setting and wider clinical teams.
Serve as the main point of contact for tertiary centres repatriating patients.
Qualifications & Requirements
Excellent communication skills.
Ability to problem solve and coordinate patient care within the acute setting.
Experience or ability to work collaboratively with diverse health and social care partners.
Registered with the PVG Scheme (Protecting Vulnerable Groups Scheme).
Note: This post is not eligible for relocation expenses.
