Host Home Contract Monitor - Eastern Region
LSI is currently offering a sign-on bonus up to $500 for full-time Host Home Contract Monitor. This role is a dedicated, energetic full-time Contract Monitor for our Host Home Services in the Eastern Iowa regions.
The position encompasses the following counties: Allamakee, Benton, Black Hawk, Bremer, Buchanan, Butler, Cedar, Chickasaw, Clayton, Clinton, Delaware, Dubuque, Fayette, Floyd, Howard, Jackson, Johnson, Jones, Linn, Louisa, Muscatine, Scott, Winneshiek. May require travel outside of listed counties to cover service needs and meetings.
Duties
Oversee specified Host Homes in the Eastern Iowa service area.
Act as main contact for interdisciplinary team members and consumers.
Provide support, assistance, and instruction to consumers to facilitate personal growth and independence.
Participate in interviews and evaluations related to contracting process for independent providers.
Ensure contractual compliance and that independent providers provide adequate care and meet contractual requirements.
Work long hours including some evenings and weekends; be part of an on-call rotation.
Qualifications Minimum:
HS Diploma/GED and five years of related experience OR Associate’s Degree and three years OR Bachelor’s Degree and one year of related experience.
Preferred:
Bachelor’s Degree in a human services related field and one year of related experience, and years of related experience in a supervisory or leadership role.
