Summary St. Cloud VA Health Care System seeks a Rehabilitation and Extended Care (REC) Integrated Clinical Community (ICC) Director in St. Cloud, MN. The incumbent must possess strong managerial and organizational skills and extensive clinical knowledge in rehabilitation and extended care in both inpatient and outpatient settings. The position may be eligible for the Education Debt Reduction Program (EDRP).
Qualifications
United States Citizenship (non‑citizens may be appointed only when qualified citizens cannot be recruited).
Doctor of Medicine or equivalent, from a school approved by the Department of Veterans Affairs.
Full and unrestricted medical license in any U.S. state, territory, or the District of Columbia.
Completed residency in an ACGME or AOA accredited core specialty training program; board certification preferred.
Physical requirements: successful completion of a pre‑employment examination.
Proficiency in spoken and written English.
Board certified in Internal Medicine or Family Medicine (preferred). Additional experience in Hospice & Palliative Care, Geriatric Practice, Post‑Acute and Long‑Term Care, Traumatic Brain Injury, Spinal Cord Injury, ALS, or Veteran Directed Care is a plus.
Strong leadership, coalition building, and communication skills.
Duties
Program responsibility and oversight of a 225‑bed Community Living Center (CLC) and associated inpatient and outpatient programs (palliative care, hospice, adult day health care, home and community based services, etc.).
Advance personalized, proactive, patient‑driven healthcare programs and inspire Veterans to highest levels of health and well‑being.
Coordinate and direct all administrative and clinical functions of the REC ICC and supervise physicians and managers.
Support VHA in meeting Joint Commission, CMS, and OSHA standards for post‑acute and long‑term care.
Collaborate with the REC quality team to meet accrediting body requirements and respond to internal evaluations.
Serve as a core member of the facility medical staff, attending committees and councils as a REC representative.
Provide clinical oversight to the CLC admission process and support nurse practitioners/physician assistants.
Lead quality assessment, quality improvement, and utilization management activities including infection control and hazard identification.
Provide continuing education to staff across the service.
Ensure compliance with VHA handbooks, directives, and local policy.
