Dynamic Role Responsibilities
  • Conduct comprehensive assessments of patient health status, tailoring from focused to extensive evaluations depending on the setting.
  • Collaborate with the Service Line Director and Clinic Site Director to assess and enhance patient services continuously.
  • Engage with a dynamic team in the ongoing development and implementation of clinic strategies.
  • Consult and collaborate with supervising physicians, specialists, and colleagues effectively.
  • Document patient care meticulously and promptly.
  • Provide medical services, assess, and treat clinic patients with excellence.
  • Manage patient consultations via telephone, conducting triage and referrals where necessary.
  • Review and interpret lab and test results, initiating follow-up actions for abnormal findings.
  • Refer complex cases to physicians, engage in consultation on diagnostic challenges and chronic condition re-evaluations.
  • Champion patient education in health promotion, disease prevention, and health maintenance.
  • Gather patient health histories via interviews and record reviews.
  • Order and perform diagnostic and therapeutic procedures within practice scope, including injections, CPR, and screenings.
  • Handle specimen collection for common analyses and cultures.
  • Interpret health information, crafting actionable treatment plans informed by comprehensive data analysis.
  • Prescribe necessary medications and therapeutic resources appropriately.
  • Ensure precise and timely health record entries.
  • Advocate passionately for patient rights and care.
  • Deliver primary clinical therapy to individuals and groups.
  • Engage actively in committees focused on healthcare quality improvement.
Innovative Qualifications
  • Current, unrestricted Texas Advanced Practice or Midwife license.
  • Active DEA certification.
  • Certified in Basic Life Support (BLS).
  • Proficient in Electronic Medical Records (Epic experience preferred).
  • A minimum of 2 years of experience as an Advanced Practice Provider required.
  • Bilingual in Spanish is preferred.
