Posting Summary Rutgers, The State University of New Jersey, is seeking a Health Care Case Manager for Mercer South for the Child Health Program within the School of Nursing. The Health Care Case Manager (HCCM) functions to support the Department of Children and Families (DCF)/Child Protection and Permanency (CP&P) in meeting the health care and protection needs of children in out‑of‑home placement, in collaboration with other staff of the Child Health Unit in that local office.
Responsibilities
Upon entry into out‑of‑home placement, assists with enrollment of a child in a managed care entity and identification of a medical home (primary care provider), and informs the child, caregiver and DCF/CP&P, as needed.
Assesses a child’s health care status and needs by retrieving a complete health history through obtaining and reviewing all medical records and interviewing past providers and caregivers; forwards all information to the medical home and other health providers as needed.
Schedules an Initial Physical Health Evaluation, in consultation with the caregiver and DCF/CP&P, reviews the preliminary and final reports, and ensures that all recommendations for health care are completed.
Develops a child health care plan in collaboration with the child’s medical home, as well as involved specialists, caregivers, other community service providers, and when appropriate, DCF/CP&P.
Links the child and caregiver with appropriate health resources and community services.
Assures that the child is scheduled for and attends timely EPSDT visits and specialty appointments, reviews all reports, and ensures all necessary reports are forwarded to the medical home.
Qualifications Minimum Education and Experience:
Current licensure as a Registered Nurse in the State of New Jersey required.
Bachelor’s Degree in Nursing or a BA/BS degree in a health‑related field required; may substitute years of pediatric nursing experience for degree.
Minimum of 1‑2 years of full‑time clinical experience as a pediatric nurse in pediatric environments such as pediatric doctor’s offices, pediatric hospital unit, pediatric public health nursing, pediatric emergency department/pediatric critical care unit, family practice offices, school nurse, child welfare nursing, pediatric/junior mental/behavioral health facilities, or children’s hospital.
Current certification in American Heart Association Basic Life Support for Healthcare Providers required.
Vehicle and valid New Jersey license required for off‑site visits.
Preferred Qualifications:
Two (2) years of experience in nursing case management, community health nursing, or nursing care of vulnerable families preferred.
Physical Demands and Work Environment
Standing, bending, and walking (up and down) stairs and the ability to lift 50 pounds (infants/children records) to waist level.
