Transportation:
Must have a valid SC driver’s license, reliable transportation, and provide proof during the interview.
Summary
Position type: Non‑exempt, full‑time or part‑time, hourly role.
Scope of role: Provide direct support within a single program or unit, typically supporting 4–6 individuals in residential settings or up to 10 in periodic programs.
Key responsibilities:
Deliver support per individual service/program plans, including socialization, health maintenance, medication administration, and skill/behavioral development.
Assist with daily activities: personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other activities of daily living.
Essential Job Functions
Direct support: Assist with socialization and behavioral development, personal care, housekeeping, recreational activities, transportation, community orientation, shopping, financial management, citizenship, and other daily activities.
Records: Accurately document progress and activity; maintain confidentiality; review records and logs for updates to service plans.
Management of individuals’ assets: Make cost‑effective purchases within the budget and according to the individual’s choices.
Relationships: Maintain professional relationships with individuals, families, guardians, and case managers; greet visitors; implement Sevita Customer Service Standards.
Reporting: Report any instance of alleged abuse or neglect per internal and external standards; report medical, behavioral, and other incidents per policy.
Rights: Maintain confidentiality; respect the rights of persons served; practice universal precautions; assist individuals in exercising their rights.
Health care: Accompany to medical appointments; administer or support medication administration; monitor health; communicate with nurses or supervisors; call 911 in emergencies; follow health directives.
Meal preparation: Prepare or support food preparation per menus and choices; monitor meals; implement dining plans.
Employee scheduling: Work scheduled hours, arrive on time, stay until replacement arrives, notify supervisor when unable to work, and fill in shifts when needed.
Teamwork: Work collaboratively, value contributions, communicate openly, listen, and show respect.
Workplace safety: Comply with safety policies, report hazards, and participate in safety training.
Maintenance: Transport individuals safely; perform housekeeping; minor repairs; yard maintenance; report equipment or facility issues.
Other duties: Perform additional required duties; use Mobile Punch for timekeeping.
Supervisory Responsibilities

Minimum Knowledge And Skills Required
High school diploma or equivalent preferred.
Six months of human services experience preferred.
Must be 18 years of age.
Working knowledge of computers.
Certificates, Licenses, And Registrations
Valid driver’s license in good standing.
Car registration and vehicle insurance if providing transportation.
All state‑required training and certification completed in mandated timeframes.
Maintain a valid driver’s license and meet all insurance requirements; approved by management to drive for the company.
Work Environment
Works at program locations and may accompany individuals into the community.
Physical Requirements
Heavy work: Exert up to 100 pounds of force occasionally, up to 50 pounds frequently, and up to 20 pounds constantly to move objects.
