Summary
Pay Class: Full‑time
Hours Available: 2nd shift (including every other weekend)
Location: West Jacksonville, FL 32244
Rate of Pay: $15.00 per hour
Position Type: Non‑exempt, full‑time or part‑time, hourly role.
Key Responsibilities
Deliver support per individual service or program plans, including socialization, health maintenance, medication administration, and skill/behavioral development.
Assist with daily activities: personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other activities of daily living.
Essential Job Functions
Perform each essential function listed below satisfactorily.
Services and Supports
Direct support in accordance with service or program plans.
Accurate documentation of progress and activities.
Manage individuals’ assets cost‑effectively within budget.
Maintain professional relationships with individuals, families, guardians, and case managers.
Report abuse/neglect, incidents, and medical events per policy.
Safeguard confidentiality and respect the rights of those served.
Health Care
Accompany individuals to medical appointments.
Administer or support administration of medication and document delivery.
Monitor health concerns, document, and communicate with supervisory staff.
Use medical equipment safely and report malfunctions.
Prepare and support meal preparation in accordance with menus and individual choices.
Employment Responsibilities
Attend orientation, ongoing training, and monthly staff meetings.
Adhere to scheduling, arrive on time, and notify supervisor of absences.
Demonstrate teamwork, courtesy, and respect toward colleagues.
Maintain a safe workplace, comply with safety policies, and report hazards.
Maintenance
Transport individuals safely, observe local laws, and maintain safety equipment.
Perform daily and seasonal housekeeping and report hazards.
Monitor and perform minor repairs and maintain facilities.
Check water temperature when assisting with bathing and participate in safety drills.
Other
Perform additional duties and activities as required.
Use Mobile Punch for timekeeping.
Supervisory Responsibilities
None
Minimum Knowledge and Skills Required Education and Experience
High school diploma or equivalent (preferred).
12 months of experience in human services with a behavioral focus (required).
Minimum 18 years of age.
Working knowledge of computers.
Certificates, Licenses, and Registrations
Valid driver’s license in good standing.
All state‑required training and certification completed within mandated timeframes.
Maintain valid driver’s license and meet insurance requirements to serve as a company driver.
Work Environment
Work at program location and accompany individuals into the community.
Physical Requirements
Occasional lifting up to 100 pounds; frequent lifting up to 50 pounds; constant lifting up to 20 pounds to move objects.
