Licensed Clinical Social Worker (LCSW)
Azza HealthCare Agency is hiring a Licensed Clinical Social Worker (LCSW) specializing in geriatric behavioral health to support long‑term care facilities across multiple Midwestern states. The position can be part‑time or full‑time and is primarily based in Fond du Lac, WI, with a flexible daytime schedule (Mon‑Fri).
Responsibilities
Provide direct psycho‑social services, including psycho‑educational support, skill building, and patient advocacy for nursing home residents.
Conduct comprehensive evaluations and diagnoses, develop and implement individualized, group, and family therapies.
Collaborate with an interdisciplinary team to ensure high‑quality, evidence‑based care.
Maintain accurate clinical documentation and comply with Medicare guidelines.
Serve approximately 10–15 patients per day with flexible scheduling.
Qualifications
Valid LCSW license in the state of Wisconsin.
1–2 years of experience in geriatric or behavioral health settings, preferably in nursing homes.
Proficiency in electronic health record technology and documentation systems.
Excellent communication and interpersonal skills.
Knowledge of Medicare billing and compliance standards.
Ability to see 10–15 patients daily and adapt to flexible scheduling.
