Active, unencumbered LPN/LVN license in the state of practice
Compact licenses transferred within 90 days of state of residence
Current CPR certification (hands‑on component) – Aveanna may assist after hire
TB skin test within last 12 months
Six months prior hands‑on nursing experience preferred, not required
Reliable transportation
