In this role, you will ensure the delivery of high-quality patient care by adhering to care plans and supervising nursing personnel, including Licensed Practical Nurses (LPNs) and Certified Nursing Assistants (CNAs). Key Responsibilities: - Conduct ongoing assessments of patients, addressing their physical, emotional, social, educational, and functional needs.- Manage patient admissions and transfers, completing a nursing database within 24 hours.- Develop and implement individualized nursing care plans based on assessment data, including nursing diagnoses, interventions, and discharge planning.- Evaluate patient safety and fall risk levels, providing necessary education and counseling to patients and families.- Participate in interdisciplinary team meetings to assess and evaluate rehabilitation goals.- Accurately transcribe and process medical orders and integrate the medical care plan with nursing care.- Administer medications and IV therapy per established policies, including evaluating the effectiveness of PRN medications.- Handle emergency situations promptly and efficiently, using skills to initiate CPR and manage Code Blue or Medical Emergency situations.- Maintain complete, factual, and timely documentation in patient medical records, including discharge instructions and interagency transfer forms.- Perform comprehensive assessments and promptly act on changes in patient conditions using the SBAR communication method.- Validate lab and test results, notifying physicians of any critical findings, and provide wound care in accordance with policies.- Supervise and delegate tasks to nursing aides and LPNs, ensuring appropriate completion of duties.- Engage in quality improvement activities and continue professional development to enhance nursing skills. Qualifications: - Active Registered Nurse (RN) license.- Basic Life Support (BLS) certification.- Advanced Cardiovascular Life Support (ACLS) certification.- Minimum of two years of bedside clinical experience is highly desired.
