Description Conduct thorough evaluations of patients in both inpatient or outpatient settings, crafting tailored treatment plans based on individual needs Implement physical therapy interventions concentrating on mobility, strength, balance, and overall function Collaborate with a multidisciplinary team including nurses, physicians, case managers, and other rehab professionals to maximize patient outcomes Provide education to patients and their families regarding therapeutic objectives, discharge planning, and prescribed home exercises Document evaluations, treatment progress, and outcomes adhering to facility and regulatory standards Engage in care team meetings and interdisciplinary discussions to guarantee coordinated and effective patient care Education
Doctorate in Physical Therapy (DPT) or equivalent qualification from an accredited institution Certifications
State licensure as a
Doctor of Physical Therapy CPR certification preferred Specialty certifications (e.g. neurological, geriatric) are advantageous, though not mandatory Skills
Previous experience is highly desirable Ability to manage patients with intricate medical and mobility challenges Strong teamwork and communication abilities Familiarity with digital charting and clinical record-keeping systems Benefits
