Eventus WholeHealth delivers an integrated model of care to adults residing in skilled nursing and assisted living facilities through a network of healthcare providers, including Physicians, Nurse Practitioners, Physician Assistants, Medical Assistants, and in-house Support Staff. Essential Duties and Responsibilities:
Provide primary-care services in accordance with site-specific protocols and the standard of care for family medicine, geriatric medicine, and/or internal medicine related to long-term care facilities. Quality/Compliance:
Review and sign MARs, sign all orders specific to Primary Care within 24 hours, manage labs and narcotic prescriptions per policy, and provide administrative documentation within specified timeframes. Maintain open communication with facility staff and administration, providing instructions and feedback as needed. Meet with facility care coordinators daily. Adhere to Eventus WholeHealth standards, policies, procedures, and all legal and regulatory requirements, including HIPAA compliance. Maintain patient confidentiality at all times, including proper use of communication tools and documentation. Consult with the clinical supervisor before refusing resident care requests. Qualifications/Education:
Master's degree in Nursing from an accredited institution and a valid license to practice medicine in your state, along with current certifications. Or, graduate of an accredited Physician Assistant program, passing the national certification exam, licensed as a Physician Assistant in your state, with preferred specialization in geriatrics, family, or psychiatry.
