Under the direction of a program manager, conduct on-site licensure inspections, complaint and incident investigations, and annual comprehensive surveys in facilities to determine compliance with federal and state health and safety regulations. Complete inspection or survey reports detailing deficiencies. Communicate clearly and accurately with internal and external stakeholders to provide, exchange, or verify information, address inquiries or complaints, and ensure services meet residents’ specialized needs. Conduct exit conferences with facility staff regarding findings. Visit long-term care facilities to verify correction of previously cited regulatory violations. Perform quality assurance reviews of documents such as publications, assessments, and reports. Testify at hearings or in court proceedings as needed to provide information on policy, procedures, history, findings, or activities. Complete referrals for conduct, as appropriate. Be available for on-call duties for emergencies (e.g., weather events) or high-priority situations in facilities or at providers. If the employee is a current registered or licensed nurse, there is potential for skilled care. Knowledge, Skills And Abilities
Ability to interpret policies, procedures, and regulations. Ability to work independently and use computer for writing reports. Ability to exercise professional judgment and reach sound decisions. Ability to integrate complex regulatory concepts, procedures, or processes. Ability to multi-task and track multiple activities with competing priorities. Registrations, Licensure Requirements Or Certifications
Surveyor Minimum Qualifications Test (SMQT) Certification or obtain within 12 months of hire. Licensed to practice as a social worker by the Texas State Board of Social Work Examiners. Initial Screening Criteria
Experience in assessment and documentation of social service needs. Experience working as a social worker in an acute or long-term health care setting preferred. Additional Information
Flexibility in work hours; travel up to 80% statewide, including overnight and weekend travel. On-call duties may be required. Reliable transportation is required.
