Conduct adult / geriatric assessments including medical history, diagnosis, treatment, health education, referrals, and documentation per Matrix guidelines Administer and order point‑of‑care and lab‑based testing as needed Collaborate with physicians, case managers, social workers, family members, and ancillary medical personnel Work with Primary Care Physician on patient education and follow‑up Provide services at the member’s residence or skilled nursing facility Benefits

Flexibility – schedule visits based on your availability (Sunday‑Saturday, 8 am‑7 pm) Competitive Pay – $90‑115 per assessment; $1,800‑$11,500 per month based on visit volume Mileage reimbursement, 401(k), employee referral bonuses, and limited voluntary benefits Education and Support – onboarding training, a preceptor, 1:1 feedback, and free continuing education access Additional Licensure – support for obtaining state licensure and credentialing in neighboring states Full‑time Opportunities – PRN providers can transition to full‑time roles with sign‑on bonus Qualifications

Master’s Degree or commensurate experience and NP licensure Current NP licensure in state of practice including prescription authority AANP, AACN, or ANCC board certification in an accepted specialty Current BLS, ACLS or CPR certification One year NP experience preferred; new grads encouraged Valid state driver’s license and proof of automobile insurance Strong computer skills and familiarity with health/medical record software Excellent verbal and written communication skills Company Culture
